An application for approval of a plat of consolidation shall be filed with the plat
officer on a form provided by the village and shall have the form and content provided
in section 22-56 of this chapter for final plats of subdivision; provided, however, that the signature
blocks for village officials, as required by subsection 22-56(18), shall be in a form satisfactory to the plat officer and shall read substantially
as follows:
This plat has been reviewed in accordance with the procedures and standards required
by the Libertyville Subdivision Code and has been found to be in compliance with such
procedures and standards.
Such applications shall be accompanied by payment of the fees and escrow deposits
required in the annual fee ordinance.
